SAIL Waiver Participants / Employers

The 21st Century Cures Act, Section 12006 , requires States to implement Electronic Visit Verification (EVV) for Medicaid-covered Personal Care Services.

The SAIL Waiver deadline to do this is August 16, 2024.

SAIL helps Alabamians with the most-significant disabilities achieve their independence. With compassionate, skilled and trusted counselors, nurses and specialists, SAIL provides in-home education, counseling, attendant care, training, medical and vocational services to ensure independent living can be achieved at home, at school, at work and in the community.

No matter the severity of a person’s disabilities, SAIL makes every effort to provide the support and services they need to live independently or to engage in employment consistent with their capacities and abilities if they want or need to work. SAIL’s counselors serve as the family’s one point of contact to coordinate combined services for Homebound/Waiver and Vocational Rehabilitation consumers.

SAIL also advocates and strives for a higher quality of life for its consumers by individualizing their services, helping individuals and families adapt to greater independence and building confidence in their abilities. It’s this comprehensive and personalized approach that helps ensure our consumers achieve their maximum potential.


SAIL provides services for Alabamians with the most significant disabilities through specialized programs that promote independent living, enhance the quality of life and maintain maximum independence in the consumer’s home:

  • Nursing management
  • Guidance and counseling
  • Patient and family education
  • Attendant care
  • Assistance with disability-related medical supplies/medications
  • Prescribed medical equipment
  • Limited home modifications
  • Referral to other services

SAIL provides supports to help eliminate barriers that prevent consumers from functioning independently and being employed:

  • Home modification assistance
  • Information and referral
  • Locating and acquiring resources, supplies and equipment
  • Advocacy
  • Guidance and counseling

SAIL provides supports in the consumer’s home, school, and/or workplace, reducing the number in institutional placements:

  • Case management
  • Personal care
  • Evaluation for assistive technology
  • Prescribed assistive technology
  • Prescribed medical supplies
  • Personal Emergency Response System (PERS)
  • Environmental modifications
  • Prescribed minor-assistive technology
  • Personal assistant services (for those employed at least 40 hours per month)
  • Service coordination
  • Referral services
  • Respite Care

SAIL provides global supports to assist consumers in reaching employment goals. All services are provided through Homebound or Waiver programs:

  • Vocational evaluation
  • Counseling and guidance
  • Training
  • Advocacy
  • Job development and placement


The eligibility criteria are slightly different for each program:

Homebound Services

  • Be an Alabama resident
  • Be at least 16 years of age
  • Have a medical diagnosis of traumatic brain injury and/or quadriplegia
  • Be dependent on others for assistance with 6/8 activities of daily living
  • Demonstrate financial need

Home and Community-based Waiver Services

  • Be 18 years of age or older with onset of disability prior to age 63
  • Experience a physical disability resulting in nursing facility level of care
  • Meet Medicaid financial eligibility

Community Support Services

  • Be an Alabama resident
  • Be severely disabled
  • Have a disability that limits the individual’s ability to maintain independence
  • Have evidence that the receipt of services will improve the individual’s ability to independently participate in their community

SAIL/VR Hybrid Services

  • Meet eligibility criteria for Homebound or Waiver Services
  • Have a reasonable expectation that service provision will result in an employment outcome
